Centurion Foundation Documents & Forms
For the 2023-2024 school year, CF has new forms and procedures. Many forms are now submitted on-line; not scanned or emailed. Please review the Quick Reference Guide and Procedures Manual for more detail.
Quick Reference Guide (new for 2023!)
Centurion Foundation Donation Form
Booster Club Agreement (new for 2023) – Booster Clubs must sign this form to indicate they they have read the Procedures Manual and will abide by Centurion Foundation Rules and Regulations.
Fundraising Request Form (2023) – Always start with this form if you are raising/receiving money that will be deposited into your CF account (as opposed to ASB account). (Spirit wear, pancake breakfasts, donation letter to parents etc.)
Deposit Verification Form (new for 2023)– Use to record bank deposits. This is an online form and submitted online. If there are questions or issues accessing the form, email treasurerliaison@uchscf.org.
Cash Verification Form (new for 2023)– Used to verify cash activity for deposits. Required to be filled out, signed and uploaded when submitting the Deposit Verification Form when cash is involved.
Expense Reimbursement Form (new for 2023)- Used to request reimbursement for expenses. This is an online form and submitted online. If there are questions or issues accessing the form, email treasurerliaison@uchscf.org.
Matching Funds Notification Form – (new for 2023) – Use this form if you are requesting matching funds from your employer, so CF can verify the donation and track the matching check, ensuring it reaches the correct club account. This is an online form and submitted online.
Donation Templates
Customize these Microsoft Word templates for several donation activities and submit with CF Fundraiser Request to clubliaison@uchscf.org three weeks prior to distribution. Centurion Foundation must approve all letters prior to distribution.

UCHS & District Forms
Activities Approval and Facilities Reservations – Use this form if you are using a school space for your event. This form is sent to the school’s Admin Assistant and it will be put in the school master calendar (media center for a meeting, activity room for practices etc.)
Rental Request Directions – Also use this form if you are using school grounds for a fundraiser outside of school hours only. The district may impose a “rental fee” (depending on the nature of the activity), which is required to be paid directly to them prior to your event.
2023-2024 Athletic Packet – In order to be eligible to try out and participate on a sports team, every student-athlete must first complete the athletic clearance process each school year. Details are available on the UCHS Athletics Webpage
