2020-2021 Athletics Information and Physical Packet

As of 6/30/20 the district has released the physicals packets for the 2020-2021 school year. Students can find the forms at the Athletics Information website here: https://uchscenturions.com/2020/06/10/2020-2021-athletics-information/

Information about fall sporting teams is scheduled to be released on July 20, 2020. According to the district, we are not sure whether or not we will have sports in the fall, but we need to start the clearance process now in order to be ready in case sports are allowed.

Centurion Foundation Grants 2019-2020

Does your sports team or club need some extra funding? The Centurion Foundation (CF) may be able to help! The CF is a non-profit 501(c)(3) organization dedicated to supporting the UCHS Athletics Teams, Clubs, Students and Families. Through the Grant Program, the CF is able to provide monetary support for needs that may not currently be available in your budget.

The CF is able to offer this assistance as a result of direct donations and fundraising activities. We hope that each of you will join and urge others to support the Centurion Foundation so that we can continue to award these CF grants.

To apply for a grant from the CF, your Team, Club or Department must be an active participant with the Centurion Foundation. An Active Participant is defined as a minimum of $100 in the Centurion Foundation account and your liaison, head coach or club advisor must attend at least one CF Board meeting in the current school year. We have 3 granting cycles. Grant period applications are due on the following dates:

  • Submission Deadline:
    • Fall 2019: Due Wednesday, October 2, 2019
    • Winter 2019/20: Due Wednesday, December 4, 2019
    • Spring 2020: Due Wednesday, April 1, 2020

Please scan and email the grant form to grants@uc-centurionfoundation.com.

Groups will be notified after Friday, October 11th. We look forward to reviewing your grant application. All grants will be submitted for final approval by the Foundation Board. Please email grants@uc-centurionfoundation.com if you have any questions about the grant process. No late submissions will be accepted.
